- Original passport must be valid for at least 6 months - beyond intended stay duration with minimum 2 blank adjacent (facing) visa pages (note: the amendment pages at the end of the passport - do not count as Visa pages)
- Visa application form duly filled, PRINT & SIGN
- Every section of the application form must be completed.
- Incomplete forms will be subject to delay
- Two (2) recent photographs
- passport size. (2 x 2 in)
- Photos must be recent, frontal-view, of the same batch, untouched, taken on white background
- showing applicant wearing no glasses of any kind, except for prescription glasses
- Please write first and last names of applicant in pencil on back of photos
- A copy of the travel itinerary
- Additional requirements for non-US citizens:
- Copy of original passport
- Copy of original or valid alien registration card or valid US resident Visa.
Time Taken: 3 business days
Visa Processing Fee: $ 60 (Single entry - 30 day validity); $ 90 (Double entry - 60 days validity);$ 90 (Multiple entry - 60 days validity)
$ 30 (additional fee for Same Day processing); $ 20 (additional fee for Next Day processing)

- Copy of valid International Vaccination Certificate (Yellow Fever mandatory, recommended Cholera, Hep A, and Malaria prevention pills
